The overall goals of our Case Management Team are to empower families to achieve their goals, improve their quality of life, and enhance their overall well-being. Case Managers work closely with DFPS, HHS, YFT, Superior Healthcare, PCS service providers, and the Treatment plan to identify needs, develop a plan of care, coordinate and monitor services, and evaluate the outcomes of their interventions.
